Hyperchloraemia in sepsis

What is the cause of hyperchloremia in Sepsis? Is it normal saline resuscitation? May be, but not always. There are indications that hyperchloremia in sepsis is part of the pathophysiology.

 

Hyperchloraemia and mortality in sepsis:

-Higher chloride level is negatively associated with survival.

 

Effects of hyperchloraemia on the gastrointestinal tract: complications such as gastroparesis, emesis and altered intestinal permeability.

 

The latest Surviving Sepsis Campaign guidelines do not recommend balanced crystalloids over saline; however, close monitoring of chloride levels is advised for avoidance of hyperchloraemia.

 

Balanced crystalloids appear to improve the sepsis outcome, when compared to saline particularly if the patient is more sick.
